Our company is currently required to conduct an assessment of the current status of occupational disease hazards and plans to select an evaluation service provider through a request-for‑quotation procurement process. We hereby invite your company to participate in the bidding for this evaluation service and kindly request that you submit a quotation for the fees associated with the project’s evaluation services.
I. Project Overview:
1. Project Location: Within the premises of Chongqing Suote Salt Chemical Co., Ltd., No. 98 Longxiang Avenue, Wanzhou District, Chongqing.
2. Project Overview: The salt‑production area, the cogeneration area, the salt‑packaging area, and auxiliary utility facilities (including dining facilities, restrooms, power supply, gas supply, and other supporting infrastructure).
II. Qualification Requirements:
Qualification requirements for assessing the current status of occupational disease hazards: Occupational health technical service institutions must hold qualifications whose scope of business covers the chemical, petrochemical, and pharmaceutical industries.
III. Quotation Method and Requirements:
1. This evaluation service fee quotation adopts a one-time pricing mechanism, and the total quoted amount includes both the expert review fees and applicable taxes.
2. The quotation documents must include:
“Quotation Letter”: All documents, including your company’s business license and copies of qualification certificates, must be stamped with the company’s official seal.
3. The estimated project duration is 30 calendar days.
4. No advance payment will be made for this project; full payment will be made in a single installment upon receipt of the official report.
V. Principles for Selecting the Evaluation Service Provider:
Provided that the qualifications and project schedule meet the requirements, the lowest tax‑exclusive bid shall be the sole criterion for selecting the service provider (with no additional fees payable), and the contract shall be awarded to the lowest bidder.
