Our company is currently required to conduct an assessment of the current status of occupational disease hazards and plans to select an evaluation service provider through a request-for‑quotation process. We hereby invite your company to participate in the bidding for this evaluation service and kindly submit your quotation for the project’s evaluation fees.
I. Project Overview:
1. Project Location: Within the premises of Chongqing Xiangyu Salt Chemical Co., Ltd., No. 98 Longxiang Avenue, Wanzhou District, Chongqing.
2. Project Overview: The gasification area (including the coal yard, coal handling, and air separation sections), the ammonia synthesis area, the combined alkali production area, the alkali fertilizer packaging area, the storage and transportation area, and the auxiliary utility facilities (such as dining facilities, restrooms, power supply, gas supply, and other supporting infrastructure).
II. Qualification Requirements:
1. Qualification requirements for assessing the current status of occupational disease hazards: The occupational health technical service agency must hold the relevant qualifications, with a scope of business covering the chemical, petrochemical, and pharmaceutical industries.
2. Possess the professional competence to conduct occupational hazard assessments; have completed at least one comparable assessment project within the past two years, and provide scanned copies of the relevant contracts.
III. Quotation Method and Requirements:
1. This evaluation service fee quotation adopts a one-time pricing mechanism, and the total quoted amount includes both the expert review fees and applicable taxes.
2. The quotation documents must include:
“Quotation Letter”: All documents, including your company’s business license and copies of qualification certificates, must be stamped with the company’s official seal.
3. The required evaluation period is 30 calendar days.
4. No advance payment will be made for this project; full payment will be made in a single installment upon receipt of the official report.
V. Principles for Selecting the Evaluation Service Provider:
Provided that the qualifications and project schedule meet the requirements, the lowest tax‑exclusive bid shall be the sole criterion for selecting the service provider (with no additional fees payable), and the contract shall be awarded to the lowest bidder.
